Enrollment & Admissions
The Alabama Department of Health requires that all enrolled children have an up-to-date record of immunization on file at the Day School. The Alabama Department of Human Resources monitors student immunization records. The Day School will track expiration dates of immunization certificates and will remind parents to collect a current immunization certificate from their child’s physician when necessary. First United Methodist Church Day School does not accept religious exemptions in lieu of a certificate of immunization. We will not accept families unwilling to adhere to these immunization schedules.

A Note About the Child’s Preadmission Record DHR-CDC-739
Please ensure this form is filled out in its entirety, including addresses and phone numbers for all emergency contacts and authorized pick-up persons.
A completed form will include five parent or guardian signatures. This form must be submitted with your student’s registration paperwork.

- Activities Away from the Facility: Pre-K & TK students participate in local field trips. Toddlers and preschoolers may enjoy short walking trips or stroller rides. Parents are notified in advance and invited to attend. If marked “No,” your child must stay home or be signed out and transported by an adult.
- Transportation: Church buses are used for Pre-K & TK field trips when a parent is not attending. Children under age 5 must have a parent install a 5-point harness car seat on the morning of the trip. This permission also applies to stroller rides for infants and toddlers.
- Water / Wading Activities: While we do not offer swimming, children may participate in water play (sprinklers, baby pools, water tables). This must be marked “Yes” for participation.
Please note: Students may not attend school without a completed Child’s Preadmission Record (DHR-CDC-739) on file.
